The form is here to help coordinate a service request, not to collect more information than the job needs.

What you share

The inquiry form asks for your name, email, city or ZIP code, service or league interest, and request details, with an optional phone number and preferred date. Basketball sign-ups also ask whether you are an individual player or team organizer, with an optional age group of interest. Team organizers can share a team name and estimated player count. A parent or guardian should submit for anyone under 18; do not include birth dates or a full roster of player contact details. Please leave out payment information, government IDs, access codes, and other sensitive information.

Who receives it

Requests are routed to Structural Plasticity to coordinate with Mohamed. The details needed to respond and plan the job may be shared with Mohamed. Submitting the form gives permission to contact you about that inquiry.

How it is sent

FormSubmit processes the submission and forwards it by email. Its documentation states that submissions are retained in its archive for 30 days. Email copies may remain in the recipient's mailbox. Read FormSubmit's privacy policy and submission documentation for its terms.

What this site stores

The site does not create customer accounts or save inquiry details in browser storage. The hosting and form services may process technical request data. No advertising analytics have been added.

Questions or changes

Use the inquiry form to ask about information previously submitted or request a correction or deletion. Identify the earlier request without adding sensitive details.

Bookings and imagery

Sending a request is not a reservation, league placement, or service agreement. Scope, pricing, timing, and applicable terms must be confirmed directly. Service images are illustrative; ask for current equipment and rental details.
